Your Questions
All of Your Questions Answered
Like any major change in life, the decision to move to a retirement community raises many questions. Here are answers to those we are asked most often.
You are purchasing a contract which entitles you to occupy an independent living apartment unit of your choosing while enjoying access to convenient healthcare services throughout your life. There are two contract options. The first, known as “Life Care,” has more services bundled within it. The second contract is a “Fee-for-Service” approach giving you the opportunity to “pay as you go” should you become ill and in need of long-term care. Either contract will include such additional benefits as one meal each day, housekeeping, utilities, maintenance, use of campus common areas, activity centers, scheduled transportation and invitations to in-house events.
Our application process starts by simply selecting an apartment model, completing both a financial and personal statement and offering a 100% refundable deposit applicable to your entrance fee. Additionally, your physician will need to complete a medical form and return it with the results of recent medical tests. Once your financial paperwork is approved, you will be scheduled for a non-invasive medical evaluation in The Evergreens Medical Clinic.
If you are selecting an unoccupied apartment we ask that you complete your closing within 60-90 days. If your circumstances dictate that you need to move sooner, we will make every effort to meet your timeline.
By fully completing our application process and submitting a 100% refundable deposit, you are not only positioning yourself for a future move, but you are opening the door to many membership benefits. As your apartment selection becomes available we will work down the list, ultimately offering you the opportunity to move forward on that style apartment. Should you choose to accept, we will schedule a contract closing date. Should the timing not be quite right, your place on the list will be secure. Additionally, you will receive benefits including dinner and brunch certificates, invitations to in-house events and so much more. The Priority Wait List Club allows you to enjoy some of the perks of The Evergreens now, while most importantly, securing your future home for later.
The best way to approach downsizing is to break the process into manageable pieces. Start by selecting furnishings you treasure and would hope to bring to your new home. Once you have made your choices, it will be easier to decide what to give to family, what to sell and what to donate. The Evergreens works closely with a professional senior move manager who can help you design your floor plans and take charge of the packing, moving and even unpacking of your precious belongings.
A key benefit of independent living apartments at The Evergreens is that they are designed with ample closet space. Your contract also entitles you to unlimited use of an oversized storage unit located on the same floor as your apartment at no additional cost. From the studio on up, there is plenty of space to stash your “stuff.”
We don’t just say “Welcome to The Evergreens.” We live it. Our resident welcoming committee is dedicated to offering several weeks of orientation, mentoring, companionship and assistance as you settle into your new home. And because our staff works where people live, we take great pride in helping new residents feel at home as they acclimate to our community.
Part of the fun of a move is making your new home just that, yours. The Evergreens offers a wide selection of choices in countertops, cabinetry, appliances and carpets that let you tailor the place you live to fit the way you live. You are also welcome to customize apartments with paint colors, wallpaper, built-ins, window treatments, etc.
Absolutely. The Evergreens offers you a home base with built-in healthcare security, while allowing you to continue living the lifestyle you have always enjoyed. Many of our residents maintain vacation homes and travel frequently, knowing that while away they are still covered by the benefits provided by their Evergreens contract.
Guests of our residents are always welcome at The Evergreens. Feel comfortable inviting your friends and family to stay with you in your apartment for up to two consecutive weeks, or you may choose to make a reservation in one of our guest accommodations.
Certainly. The Evergreens is very accommodating to cats, dogs and other domestic pets, with a one-time registration fee and nominal monthly charge per pet.
Your independent living apartment is equipped with a wireless “Guardian” system, emergency pull cords and a hard-wired emergency telephone button. Activating any of these will call for both security and medical help.
While your apartment and lifestyle at The Evergreens are nothing short of first class, the behind-the-scenes amenity of top-ranked medical care on premise is the real luxury. With the Van Duzer Pavilion offering residential healthcare suites and the Bishop Banyard Skilled Nursing Center, The Evergreens will provide fully accessible healthcare in a comfortable environment for temporary periods or throughout your life.
Moving to The Evergreens is a continuation of your love and caring for each other, helping to keep yourselves and your estate healthy.
One of the very best reasons to move to The Evergreens sooner than later is that arriving together makes it another adventure in your life together. There will be many new opportunities to enjoy as a couple along with personal interests and individual pursuits that you can then share with each other at the end of a busy day.
There is plenty of room for two at The Evergreens. Apartments come with ample closet space, particularly appreciated by couples. Your contract also entitles you to unlimited use of an oversized storage unit located on the same floor as your apartment at no additional cost.
Security means different things at different times. When something goes wrong in the middle of the night the value of a 24-hour emergency call service is priceless. It is a relief knowing that whatever the future may hold you have access to quality long and short-term nursing care.
Coping with health issues is stressful for both the compromised and the healthy spouse. Knowing your beloved is receiving the finest professional care just a short indoor walk away, making visits easy and as frequent as you like, is a huge relief.
Exciting times are just beginning once the decision to move to The Evergreens is made.
Deciding which floor plan and location is the perfect one for you is a process, some know right away and others return several times to reach their personal level of comfort about which to choose.
Experienced staff members in our Sales and Marketing Department are there to assist and expedite your move so you can begin enjoying the benefits and amenities as soon as possible.
Selecting your contract option is another early step, one that depends upon your insurance and financial planning preferences.
The first contract known as “Life Care” has more services bundled within it. The second contract is a “Fee-for-Service” approach that gives you the opportunity to pay as you go if you should require any future long-term care.
Either contract includes such additional benefits as one meal each day, housekeeping, utilities, maintenance, use of campus common areas, activity centers and scheduled transportation. Entertaining and thoughtful educational events enrich community life as well.
Once you have decided which apartment and contract is right for you, the actual application process begins by submitting both a financial and a personal statement along with a $500 non-refundable application fee.
Upon approval of your application, you will be asked to make a fully refundable 5% deposit that is applicable to your entrance fee.
Additionally, your physician will need to complete a medical form and return it with the results of recent medical tests. Once your financial paperwork is approved, you will be scheduled for a non-invasive medical evaluation in The Evergreens Medical Clinic.
Completing the contract for your move typically occurs within 60 – 90 days. If circumstances dictate that you need to move sooner, we will make every effort to meet your timeline.
If you see your move occurring after the 90-day timeframe, or you have decided to wait until a particular floor plan becomes available, you can join the Priority Wait List Club.
Once you have completed the application process and submitted your completely refundable 5% deposit, you can begin enjoying some of the many benefits of The Evergreens, including: dinner and lunch certificates, invitations to in-house events and more.
Most importantly your position on the Priority Wait List holds your spot, securing your future home for later.
Long-term healthcare costs can easily deplete a lifetime of savings. The Evergreens helps contain that cost and provides a framework for projecting what it could be. Knowing the limits makes financial planning for the unknown future possible.
Completing your financial application for approval is a good way to determine affordability. The Evergreens must be fiscally responsible and takes into account your assets, income, insurances, age and general health. This information will also help to determine which size apartment, or even the contract that is most appropriate for your circumstance.
In most cases, a portion of your fees at The Evergreens may be tax-deductible as pre-paid medical expenses. Because of changes in federal and state tax codes from year to year, you should consult your tax advisor to determine the extent of your allowable deductions.
There are two contract types at The Evergreens; both of them have the same services and amenities included in the monthly fee. There are obvious benefits to having things such as security, electric and one meal a day included. Potentially, real cost savings can come from included maintenance and service. All of your home and major appliance repairs, maintenance and worries are relegated to the past. Many residents eventually elect to use the included Transportation services enabling many to do without the second car or to forgo all cars, their repairs and car insurance.
The contracts specifically differ in each of the entry fees. The Life Care Contract provides a sizable discount on healthcare services should they become necessary. This extra coverage protects your estate should extended stays in long-term care be required. The Fee-for-Service Contract reduces the initial entrance fee and provides for a limited quantity of covered healthcare days if and when they become necessary. Beyond those covered days, per diem healthcare fees would apply at a 10% discount.
Both the Life Care Contract and the Fee-for-Service Contract include two financial variations – a traditional structure with a depreciating entrance fee and an alternative plan with an entrance fee that is 80% refundable. In both contracts the 80% refundable entry fee is higher than the deductible entry fee.
The traditional or deductible entry fee plan amortizes at the rate of 2% monthly for the first fifty months of your contract while living in an independent living apartment or at 4% in either healthcare area, so should you need to terminate the agreement within that time period you may recover the balance of your entrance fee.
Simply put, your unique circumstance and preferences will lead you to the best decision. Those who have Long Term Care Insurance or intend to self-insure typically prefer the Fee-for-Service contract. Those who want to leave a larger estate select the 80% refundable option of either contract.
Available assets and how much you want to allocate toward the entry fee are other financial factors to consider.
The Evergreens is a not-for-profit continuing care retirement community with a national CCAC accreditation that is awarded only to those communities who pass a rigorous internal exam and on-site survey of its program and business practices and sufficiently demonstrates its conformance to the highest standards. This meaningful accreditation gives a large measure of comfort that your loved one is receiving the best care and additionally that good business practices are being followed, an added assurance that your family is protected.
Knowing that your family member is situated someplace secure where their independent lifestyle can best be maintained for happiness, health and well-being is foremost. Socializing with friends and new neighbors at The Evergreens comes easily and fills days and evenings with interesting, fulfilling activities and conversation. There are many additional benefits that will become apparent to resident and family members alike as their new retirement lifestyle becomes familiar.
Tasks, appointments and errands are much easier to manage living at The Evergreens, particularly in bad weather. In large part because so many activities, such as banking, a beauty and barbershop, the Health Clinic, a 2500 volume library and a chapel are all there – even voting, as The Evergreens is a designated polling place.
Weekly, scheduled transportation is provided for shopping trips to grocery stores and the mall. Transportation is also provided on Sundays to the First Presbyterian Church and Our Lady of Good Counsel Roman Catholic Church. Trips to the airport or other specific destinations require an additional fee based on mileage.
Overnight visitors may be entertained in a resident’s apartment for up to three days without additional charge excepting any meals in the dining rooms. Any visit exceeding 14 days must be approved.
Private accommodations are also available for the comfort of overnight visitors. The Evergreens has special guest lodgings for family and friends at their Guest House at 313 Bridgeboro Road.
Dining is a favorite pastime at The Evergreens. Different dining venues, menus that change with the season and the occasion keep a daily event far from routine. Families are welcomed as guests with meal charges appearing on the resident’s monthly statement.
Additionally, there is a lovely, special- occasion private dining room available for reservation so that traditional family events can easily continue.
Resident Choice is a new, special system here at The Evergreens community to record each of our residents’ use of meals.
Each resident is given a unique key tag, which is swiped by the staff member in our dining department before they are seated in each of the dining areas.
Once the key tag is scanned, the information is used to track the overall use of programs, help Dining Service operations in their planning, and, most importantly, to help the Finance department track meal services for billing purposes.
Resident Choice can only be used when residents choose to eat in the Main Dining Room for lunch or dinner, and at the Atrium for a dinner buffet.
In the Main Dining Room, each resident’s key tag is scanned at the main entrance by a dining attendant before they are brought in to be seated. If a resident has guests dining with them, the appropriate charges will be recorded at that time. A paper receipt is then available upon request.
In the Atrium for dinner buffet, each resident scans their key-tag and the meal use records automatically. If a resident is in need of assistance, a dining attendant will be available to help.
For delivered meals, medical or non-medical, resident’s menu order will be completed by dining services and the meal use recorded by a dining attendant. There is no need for residents to present their key tag.
For “pick-up” meals, residents will continue to complete the ordering process either in person or over the phone. There is no need to present their key tag when picking up their meal, as a dining attendant will record each resident’s meal use.
In the Café for breakfast and lunch, residents have a choice. They can either choose to use their key-tag to charge purchases directly to their own personal Evergreens account. Or, they may choose to pay cash in the Café if they so desire.
Residents at The Evergreens can find activities to fill every day. The schedule of activities and programming also allows each person to choose what to do or what not to do.
Any resident may suggest activities through the Recreation Committee, or to the Director of Recreation and Wellness. Leadership emerges as interests develop. Opportunities abound for new experiences, but there is no pressure to participate or to abandon outside activities if the resident does not choose to do so.
The following means of communication are utilized to acquaint residents with what is happening at The Evergreens and in the community at large.
· “The Informer,” a weekly newsletter published each Saturday, presents news of all aspects of life at The Evergreens. All events are listed chronologically by day. Special upcoming events are noted on the cover page included with community announcements. Copies are distributed to all residents through the in-house mailboxes.
· Special notices and a monthly calendar of events are distributed through the in-house mailboxes.
· The Evergreens information channel, Channel 2 on residents’ television sets, carries information about activities, trips, lectures and dinner and brunch menus, as well as special announcements from Administration.
· The Evergreens Bulletin Board located on the left side of the hallway coming into the Great Hall is used for information posted by the staff of The Evergreens and the Residents Council. Resident and Community information are not to be posted on this Bulletin Board. Residents are encouraged to check the Board on a daily basis.
· The Community Bulletin Board located on the right side of the hallway coming into the Great Hall is used for flyers and material related to Community events and items posted by residents. Materials posted here must be dated and approved by the Recreation and Wellness Director. The Evergreens reserves the right to remove materials or postings that are not current, are considered offensive, or create a hostile environment, and to dispose of these materials or postings.
Residents of The Evergreens come from a variety of religious backgrounds. There are numerous churches and synagogues in Moorestown and surrounding communities. But, there is also a full-time Chaplain at The Evergreens who is responsible for conducting religious services.
The Chaplain is a priest of the Episcopal Church. There are services in the Emilie C. Williams Chapel on Sunday, and on weekdays in other areas which follow the Episcopal Book of Common Prayer. All residents are welcome to attend these services. Baptized Christians are invited to receive Holy Communion whenever it is celebrated.
The Chapel is always open and available to all residents and staff of The Evergreens. A Rosary Prayer group meets on Mondays at 2:00 PM. On the first Monday of every month, there is a Presbyterian Home Communion Service at 2:00 pm. The second Thursday of every month is a Presbyterian worship service at 11:00 am. Every third Friday of the month there is a Shabbat Service at 1:30 pm in the Van Duzer Living Room. There is a candle lighting service for the first day of Roshasana and a luncheon for Passover. The Chaplain will help to arrange for services to be conducted by clergy of other religious affiliations when this is desired.
The Chaplain is also available for spiritual counseling or guidance for any resident or family member. The Chaplain maintains a list of priests, pastors and rabbis in the area and can put residents in touch with spiritual counselors from their own faith communities.
The Chaplain may assist residents to prepare and update Advance Directives, which include a Durable Power of Attorney and a Living Will. If issues of medical ethics arise, the chaplain can assist in helping residents and families with appropriate decisions. The Chaplain can also assist with planning or conducting funerals and memorial services.
Many activities are available to residents, further promoting overall wellness and healthy living. These include, but are not limited to regular strength and stretch exercise groups, tai chi, aquacise, and open swim.
Exercise equipment is also available for use, located in the Fitness Room adjacent to the Pool. Additionally, residents are encouraged to walk outdoors or in the halls, choosing the distances most appropriate to their needs.
A fitness class called FITFUN provides residents with the opportunity to properly learn how to operate the fitness equipment in the fitness room and gain understanding of which equipment is specific and safe for their fitness level. Another class offered at The Evergreens is Tai Chi – which has also been shown to improve memory, and concentration. All classes offered have regularly scheduled sessions.
All mentioned programs improve strength, flexibility, coordination, balance and cardiovascular fitness. A medical authorization form signed by a physician is required for participation in most exercise programs.
A Game Room, located on the bottom level of the Main Building, next to the woodshop and arts and crafts room, boasts billiards and darts.
Additionally, Table Tennis can be played in LaMonte Hall and indoor shuffleboard can be played in the Arts and Crafts Room. There are also card and game rooms scattered throughout the residential ILU floors for use by residents.
For those interested in outdoor recreation, plans for a new putting green is currently underway. Horseshoes can be played by the Greenhouse, and they will be kept in the garden shed.
Additionally, there are plenty of outdoor walking paths, benches and patio spaces for residents to utilize.
Yes, The Evergreens has a library that is open for use for all of those who visit our community. The Library is attractive and restful and continues to stock various current readings, with a minimum of six best sellers at a time. Checkouts and returns are easy. There is even a children’s shelf for visiting young readers.
Like many activities at The Evergreens, the organization, maintenance and services of the Library depend on a volunteer committee (The Library Committee is a sub committee of the Resident Council Recreation Committee).
The Evergreens’ Vision Improvement Center, located diagonally across from the Library, has an Aladdin XL black and white and an Aladdin Rainbow color video magnifier which enables those with impaired vision to read and write. The SARA Reading Machine (Scanning and Reading Appliance) is also located in the Vision Improvement Center.
Residents interested in entering the Vision Improvement Center should contact the front desk who will have Security unlock the door.
Resident volunteer committees encourage groups or individuals to pursue their creative interests through various art forms and crafts. Classes in painting or drawing are arranged as desired and if interest in specific crafts is expressed, an effort will be made to secure necessary equipment and/or instruction in the Arts and Crafts Room. Residents are encouraged to brush up on old skills and to try new forms of expression.
The woodshop is a well-equipped facility available to qualified residents to pursue their interests in wood or metal working. The shop contains equipment and supplies donated by shop members as well as tools purchased with funds generated from work done for residents. Many hand tools and supplies in the shop are the personal property of shop members.
Residents wishing to use the shop on a regular basis are required to demonstrate they are fully qualified to operate any power equipment they intend to use and to agree to observe all shop procedures. Residents wishing to use the shop occasionally may do so only if a shop member is present.
The shop is financed by funds generated from the construction and repair of residents’ furniture and electrical appliances. The Shop Committee is composed of five residents which manages all shop activities.
Yes, The Evergreens has a heated indoor pool, located on the bottom floor of the Main Building, next to the Fitness Room.
Regulations regarding the safe use of the pool are posted and must be followed at all times. Children are permitted to swim on Saturdays and Sundays only and must be accompanied at all times by a resident and an adult, one of whom must be in the water. The swimming pool hosts scheduled instructed aquatic classes and times where a lifeguard is present for open pool time without arranging for a second person buddy.
A variety of aquatics classes are offered to residents, led by a certified instructor from the YMCA of Burlington County. The pool is always available to all residents when YMCA instructed member lessons are taking place.
The small gardens adjacent to the first floor apartments provide a variety of opportunities for gardeners who live in those apartments.
Behind the Guest House at 313 Bridgeboro Road, The Evergreens prepares the soil for garden plots in a fenced-in area and provides hoses as well as storage space for garden tools. These plots are assigned each spring by the Maintenance Committee in the order that requests are received.
Also at 313 Bridgeboro Road, there is a greenhouse with two large planting tables. During the winter, this heated greenhouse can be used to store plants.
For safety reasons, residents are asked to inform the front desk receptionist when you are going to the greenhouse/gardening area. The greenhouse has a Guardian System unit to accommodate our gardeners, and this unit is activated in the Security Office.
Planters to be mounted on balcony railings in the ILU apartments must be approved in writing by the Maintenance Supervisor and must be securely attached for safety.
A wide variety of programs are available for our residents’ enjoyment. These include, but are not limited to weekly films, card groups, bingo, music appreciation classes, and occasional theme events which involve games and music.
Special musical concerts and lectures of interest are also scheduled. The Recreation Committee of the Resident Council makes regular recommendations regarding entertainment to the Director of Recreation and Wellness.
The Director of Activities at The Evergreens also coordinates many of the programs available on the Van Duzer Pavilion and in the Bishop Banyard Center. Residents are invited to participate in all activities at The Evergreens.
The Director of Recreation and Wellness and the Director of Activities make up different calendars to meet different people’s needs at The Evergreens.
If you have questions about either calendar, please contact the Director of Recreation and Wellness for the Recreation Calendar at 856-439-2052 and the Director of Activities at 856-439-2080 for the Activity Calendar.
A number of opportunities for residents to learn from each other have been organized.
The Great Books Series, more recently titled Great Conversations, for example, has become a monthly feature. Video Lecture Series is also a popular study and discussion group and runs weekly according to the series topic. There are additional book discussion groups that meet regularly and others that meet from time to time for discussion on diverse topics.
All of these are open to the whole Evergreens community. Anyone can participate at any time.
Within The Evergreens there are many opportunities to assist others, especially residents of the Banyard Center and the Van Duzer Pavilion. Visiting, reading aloud, writing letters, providing transportation, all add pleasure and promote new friendships. We also have an informal volunteer program for residents called the Sharing Hearts Program.
If you are interested in volunteering or you have any family members that may be interested in volunteering, call the Recreation Department at 856-439-2052.
The Sharing Hearts Program is an informal volunteer program for the residents of the community. Individuals donate 1-2 hours per week or 1 hour per month to assist in helping, sharing and reaching out to other in the community.
This program was developed by a resident along with the Director of Recreation & Wellness. If you are interested in finding out more about the Sharing Hearts Program, please contact the Recreation Office at 856-439-2052.

Museums, historical sites, concerts, ballet, opera, dance, theater, baseball games, movies, and lectures are just some of the many field trip excursions residents of The Evergreens can enjoy throughout the year.
The Recreation Department tries to make signing up for a particular event as simple as possible. Depending on the nature of the activity, the Director of Recreation and Wellness makes inquiries and preliminary arrangements for tickets, transportation, restaurants and whatever else is needed.
Approximately a month before a scheduled event, a notice is posted telling residents how to sign up, including the cost and how to make out the necessary check(s).
Residents must sign the “Count Me In” Book at the Reception Desk to guarantee transportation.
The Evergreens has two buses, one with a twenty-passenger capacity and the other carries a capacity of 14 passengers.
For popular events, reservations are confirmed in the order they are received. It is important to note that, except for the weekly shopping trips to supermarkets and malls, there must be at least ten residents signed up in order for the trip to take place using The Evergreens transportation.
From time-to-time exceptions will be made and the transportation charge adjusted accordingly.
Residents will pay per trip charges plus ticket costs. Mileage charges will be posted on the sign-up page in the “Count Me In” Book and will be included in the resident’s monthly bill after the trip is completed.
If you must cancel your reservation, and give at least Twenty-four (24) hours notice of your cancellation, there will be no charge. Should a resident fail to cancel a trip without twenty-four (24) hours’ notice, the transportation fee for the trip will be billed to the resident.
For subscription series events, there must be a minimum of three residents for transportation to be provided. Charges will be based on a minimum of ten passengers and divided equally among those participating. Those signing for series transportation are committed to the transportation charges for the length of the subscription season. The Evergreens Administration has the sole discretion to cancel resident transportation for weather-related reasons.

Any resident may suggest activities through the Recreation Committee, or to the Event and Service Coordinator.
Materials posted here must be dated and approved by the Event and Service Coordinator.
The Recreation Committee of the Resident Council makes regular recommendations regarding entertainment to the Event and Service Coordinator.
The Director of Activities at The Evergreens also coordinates many of the programs available on the Van Duzer Pavilion and in the Bishop Banyard Center. Residents are invited to participate in all activities at The Evergreens.
The Event and Service Coordinator and the Director of Activities make up different calendars to meet different people’s needs at The Evergreens.
If you have questions about either calendar, please contact the Event and Service Coordinator for the Recreation Calendar at 856-439-2052 and the Director of Activities at 856-439-2080 for the Activity Calendar.
If you are interested in volunteering or you have any family members that may be interested in volunteering, call the Event and Service Department at 856-439-2052.
The Sharing Hearts Program is an informal volunteer program for the residents of the community. Individuals donate 1-2 hours per week or 1 hour per month to assist in helping, sharing and reaching out to other in the community.
This program was developed by a resident along with the Event and Service Coordinator. If you are interested in finding out more about the Sharing Hearts Program, please contact the Event and Service Office at 856-439-2052.
There are also numerous channels for participation in outside community activities through civic and religious organizations. The Activities Department maintains a list of needed volunteer services.
Our location affords us short, easy access to the metropolitan attractions of Philadelphia, New York and Atlantic City.
Museums, historical sites, concerts, ballet, opera, dance, theater, baseball games, movies, and lectures are just some of the many field trip excursions residents of The Evergreens can enjoy throughout the year.
The Event and Service Department tries to make signing up for a particular event as simple as possible. Depending on the nature of the activity, the Event and Service Coordinator makes inquiries and preliminary arrangements for tickets, transportation, restaurants and whatever else is needed.
Trips are made to local supermarkets, malls, the Main Street Moorestown shops and the Moorestown Library on a regularly scheduled basis. There is no charge for these trips, but residents are required to sign the “Count Me In” Book to reserve a seat on the vehicle.
The Board of Trustees of The Evergreens appoints a Medical Director responsible for the quality of health care at the facility.
The programs, services and activities at The Evergreens are planned to help residents develop and maintain healthy lifestyles. A balanced diet, regular physical check-ups and daily exercise appropriate to each resident’s needs and inclination are encouraged.
Health care is provided through a continuum of interrelated units: the Apartments, the Van Duzer Pavilion (residential health care) and the Banyard Center (skilled nursing).
The Apartments are designed for people able to function on their own. While maintenance and some housekeeping are provided by The Evergreens staff, residents plan and regulate their own time. They are free to engage in and contribute to as many or as few activities within or outside The Evergreens as they choose.
All of our residents are required to sign a Residence and Care Agreement, and are also asked at that time to make a decision regarding their primary care physician. A record of that choice must be maintained in writing with the Clinic. The Clinic must also be notified of any change in that choice immediately. That notification may be oral and followed up with written notification within seven (7) days. This written notification will be maintained with the Clinic.
Whenever a resident is admitted to a hospital, the Clinic or The Evergreens Receptionist should be notified. It is essential that those at The Evergreens who are responsible for the welfare of residents be informed. If the resident is out of state or away from the facility for an extended period of time, the Clinic must be notified within 30 days of any hospitalization which occurred during this period.
In case of hospitalization, a resident under The Evergreens “medical umbrella” (which means you have chosen the Medical Director of an associate from Evergreens medical staff as your primary care physician) will normally be referred to Cooper Hospital/University Medical Center in Camden, NJ. The Medical Director and associates are affiliated with this hospital and will provide ongoing case supervision.
Residents, whether under the umbrella or not, have the option of going to other hospitals of their choice. In an emergency situation, a resident will be taken by ambulance to the nearest hospital. Transfers may then be arranged when requested.
The Evergreens reserves the right to evaluate the resident upon his or her return to the facility.
The Evergreens has a contract with the Jersey Shore Pharmacy to serve the needs of residents and their physicians for prescription drugs.
There is always a pharmacist during business hours to answer any questions a resident may have regarding medications.
Jersey Shore accepts most insurance prescription plans. Refills are placed by the resident directly, and orders should be placed at least five days before the refill is actually needed.
New prescriptions or refills of prescriptions after the number of authorized refills has expired must be authorized by a physician. New prescriptions or reauthorizations, signed by the doctor, can be taken by a resident to the Clinic for forwarding to Jersey Shore Pharmacy or any doctor can personally order new prescriptions or reauthorize previous prescriptions by calling Jersey Shore Pharmacy directly.
New prescriptions (written) must be taken to the Clinic for forwarding to the pharmacy (if a resident uses the pharmacy under contract with The Evergreens).
If a resident’s physician has called in a prescription, they are required to notify the Clinic of this new medication.
The terms of the Residence and Care Agreement require residents to have an approved level of medical coverage, including Medicare plus secondary medical insurance. These plans differ widely because of the rapidly shifting medical insurance field and also because coverage for retired employees varies from company to company. Private health insurance also differs and can contribute to confusion among residents. Residents should check with their insurance providers to ensure compliance with the approved level of medical coverage. Any non-compliance will result in charges to be paid by the resident. From time to time, under certain circumstances, a resident may not be able to obtain medical coverage. This must be discussed with the Director of Social Services.
Residents have an obligation to immediately notify The Evergreens if their medical coverage changes. Residents are required to notify the clinic any time they change insurance coverage or receive new insurance cards. In addition, the contact names and phone numbers should be updated as needed.
The Social Services Department provides counseling and emotional support and will work with you to ensure that your personal and physical needs are met. The Social Services Department is in contact with the entire staff at The Evergreens so concerns are addressed quickly and respectfully.
The Social Services Department also manages the in-house Companion Service Program. Social Services will ensure that you understand the services provided by The Evergreens so that you feel you are truly at home. If you have any questions please do not hesitate to call us at 439-2023 or 439-2036.
