Care isn’t just what we provide. It’s what we do.
Treatment plans at The Evergreens are developed through an interdisciplinary team in consultation with residents, designated family members, and others as needed to ensure a smooth transition, if and when necessary. Our staff members, including a comprehensive Social Services Department and Chaplain, in addition to the entire medical community, are committed to serving as advocates for residents at all times.
Both residential healthcare and skilled nursing care are made available as needed to residents who may require additional medical or psychological support.
The Evergreens honors Advance Directives such as Living Wills, Instruction Directives and Durable Powers of Attorney, and encourages informed medical decision-making by our residents.
